Although most care homes provide high-quality, person-centred care, you may sometimes come across problems. It is important to keep a record of what happened.

Allowing you to provide the home with full information to help them resolve the issue, and so that you can escalate the complaint if the care home can’t resolve it itself. 

Age Scotland has produced a care home incident record template to help you to capture the important details. You can download and complete it digitally, or print it out so you can write the details in. If you would like a paper copy, please contact our helpline team, who can print and send it out to you.
